- Arrive for your appointment 15 minutes before your appointment time.
- Please bring your driver’s license, current insurance card, and co-pay (we do accept major credit cards).
- Patients 17 and under MUST be accompanied by a parent or legal guardian.
- Please bring a list of medications and all tubes of cream (over-the-counter or prescription) you have been using to treat your rash.
- If you are requesting a skin exam, please be prepared to undress and to wear one of our provided gowns. Please DO NOT wear make-up for your exam.
- If you cannot make your appointment, please give at least 24 hours notice for cancellation.
